Caterpillar Pacman Hack is part of the huge number of unofficial modifications that circulated over the years on Namco's Pac-Man (1980) source code: same maze, same ghosts, but with the protagonist redrawn as a caterpillar and some minor cosmetic tweaks compared to the original.
Like most of these hacks, it has no relation to Namco and doesn't bring substantial design changes: it's a curiosity of the ROM preservation and modification scene, one of the many disguises tried on history's most famous dot-eater.
