Fantasy is one of those generic names -also used by dozens of other games and machines in arcade history- that make it hard to trace its exact origin with certainty without direct access to the board or period documentation. The little that can be affirmed with available information is its Japanese regional classification within preservation listings.
Rather than offering data that can't be confirmed, it's worth acknowledging the limit: it's preferable to record this documentary gap than to attribute it an invented history.
